If you are interested in making a referral for yourself or another please call toll free 1-800-840-9081.
A LIFE representative will call the individual or family to explain the services and arrange an appointment to visit in the home.
LIFE staff will conduct a home visit and explain the program in depth and provide written materials about our services and benefits.
If the individual is interested in touring the LIFE Center, that will be arranged, including transportation.
If the individual would like to enroll in the LIFE program, additional assessments will be completed and a start date will be coordinated.
