LIFE (Living Independence For the Elderly) Lutheran Services is a creative program designed to enhance the ability of seniors to live safely in their homes and communities with dignity and independence by providing access to the full continuum of preventative, primary, acute and long-term care services. Known as PACE (Program of All-inclusive Care for the Elderly) nationally, LIFE includes many elements of the traditional health care system. It is coordinated by an interdisciplinary team and provided primarily in the LIFE Center, located in Chambersburg, Pennsylvania, and is supplemented by in-home and referral services according to participants' needs.
LIFE Lutheran Services has contracts with many specialists and other providers in the community to ensure that you get the care that you need.
LIFE Lutheran Services is an interdisciplinary team approach to care. The interdisciplinary team includes physicians, nurses, therapists and other health care professionals. Working together with the participant and caregiver, this team assesses the participant's needs, and using the participant's goals as a guideline, develops an individual plan of care and coordinates necessary services. Participant status is monitored and evaluated regularly and the plan of care is adjusted to meet changing needs. This model of all-inclusive care has been shown to improve participants' health status because they are so closely monitored.
