About LIFE

Eligibility & Cost

LIFE enrolls seniors who are age 55 or older, who live in a LIFE service area, need help with 3 activities of daily living such as bathing, dressing and meal preparation, and want to stay in the community. The senior must be able to live in a community setting when enrolling without jeopardy to health and safety, and agree to have all services coordinated and approved through the LIFE interdisciplinary team.

Cost

LIFE Lutheran Services receives a per participant rate from Medicare, Medicaid, and/or private payers.

• Participants with Medicare and Medicaid, pay nothing for LIFE.
• Participants with Medicaid only, pay nothing for LIFE.
• Participants with Medicare only, pay a monthly premium for LIFE.

During the enrollment process, benefits and any responsibilities will be verified. Participants may be fully and personally liable for the costs of unauthorized or out-of-LIFE program agreement services.
